The registration form is attached below as a pdf document.
In the case of candidates at 11+ and 13+, this form must be returned to the Head's PA, The Perse School, Hills Road, Cambridge CB2 8QF by the end of December in the year prior to entry. Parents of Sixth Form candidates should submit applications any time after the start of the Michaelmas Term but before the cut-off date in November of the year preceding proposed entry. Please refer to Applications and Scholarships in the current Sixth Form Prospectus for further details.
